Site of Care
In an effort to control rising health insurance premiums, throughout 2018, we worked one-on-one with some members to steer them toward more cost-effective, yet equally safe, places to receive their medication treatments.
Effective Jan. 1, 2019, we are formalizing this practice by updating our "Site of Care" language in all of our health contracts. This new language notes that a few specific drugs must be administered at one of these sites:
- Standalone infusion center
- Physician's office
- Home
- Designated hospital facility
Our case management team assists with locating approved sites of care. Learn more about case management and how their program benefits our members.
For the most current list of medications, you can review the Site of Care policy.
If you are unable to receive your medication treatment in the above mentioned sites, a prior authorization may be requested. Criteria and prior authorization forms can be found within the Site of Care policy.
Using the list below, you can search for designated hospital facilities that are covered under Site of Care.
